We pride ourselves on delivering culturally appropriate supports to enable independence and freedom to live in your community of choice. Open Minds are currently delivering its Innovate Reconciliation Action Plan since May 2016.
Includes:
- Identifying personal goals and a plan to reach them
- Assistance to do the everyday tasks like shopping and meal preparation
- Accessing local community groups and activities
- Support to use public transport, cleaning and personal care
- Allied Health referral assessment (we can recommend other avenues of health support)
- Community Re-Entry Services including accommodation
