Acquired Brain Injury (ABI) Support Services
The Acquired Brain Injury (ABI) Support Service is a specialist support unit within Open Minds delivering community based rehabilitation, support and social inclusion programs to meet the complex needs of individuals living with an ABI.
With a team of skilled staff we provide individuals with responsive and flexible community based day to day rehabilitation planning and lifestyle supports alongside and supplementary to their medical and allied health support needs. Our Individual Rehabilitation and Support Plans provide opportunities for clients and their families to establish goals and develop innovative and flexible supports to promote mental, emotional and physical wellbeing, encourage independence and social inclusion.
Open Minds works collaboratively with clients and their families, allied health professionals (e.g. physiotherapists, occupational therapists, speech pathologist, counsellors and clinical psychologists), government bodies, insurers and legal representatives to achieve optimal outcomes for our clients and their families. Our ABI service can provide services to people who: Have a primary diagnosis of Acquired Brain Injury or Trauma Are 16 years of age and above Are compensable or individually funded. The service provides programs under 3 streams: Rehabilitation Support and Coordination.
This service develops and implements recovery focused support programs based upon individual strengths, goals and resources. We understand Rehabilitation and recovery is an ongoing process. Together with clients and their families, we establish rehabilitation plans where support workers assist in therapies and skills development as well as participation in a range of activities, programs and events which promote community access, independence and achievement to enhance and retain a quality lifestyle. Lifestyle Rehabilitation Individual and innovative lifestyle support programs enhance daily living skills, promote independence and encourage community inclusion.
Programs are flexible and diverse to meet day-to-day support and rehabilitation needs and foster the client’s personal lifestyle goals and recreation choices. Daily living skills including cooking, budgeting, transport training, household routines, personal care and managing appointments can be included as well as positive behaviour planning and social interactions. The program includes a focus on sourcing employment opportunities and training to promote development of the client’s self esteem and independence.
ABI Supported Accommodation In partnership with Brisbane Housing Company, the ABI service supports a number of individuals living with an ABI in a contemporary studio apartment accommodation block with ABI specialist staff on –site 24-hours, 7 days a week. The units, located in inner Brisbane, have a strong emphasis on community inclusion and independent living. Situated within walking distance of a major shopping centre and public transport, Open Minds ABI service supports the clients as they progress through their individual rehabilitation goals, at their own pace, in a supportive real home environment. Clients are supported in a range of individual requirements including mobility, sensory impairment, cognitive impairment and mental health co-morbidity.
