2018 Queensland Mental Health Week Achievement Awards

+ Please ensure you read the Nomination Pack for evaluation criteria and how to enter.

+ Please note the conditions of entry listed in the Nomination Pack.

+ To enter the 2018 Queensland Mental Health Week Achievement Awards you must complete and sign the Nomination Form addressing the evaluation criteria.

Your nomination should:

+ Tell us why the nomination should win an award for excellence in mental health.

+ Be clear, concise, use plain language and observe word limits.

+ Focus on the results that have been achieved and their relevance to the criteria.

SECTION 3: CATEGORY NOMINATED

There are eight (8) award categories in the 2018 Queensland Mental Health Week Achievement Awards. Refer to the Nomination Pack for information to help you choose an award category.

Individual Award

Not-for-profit Organisation Award SME (1-100 staff)

Jude Bugeja – Peer Experience Award

Not-for-profit Organisation Award Large (101+ staff)

Youth Award (25 Years and Under)

Workplace Award SME businesses (1-200 staff)

Media Award

Workplace Award Large businesses (201+ staff)

Please note, if you would like to enter a nomination in more than one category, you will need to lodge a complete online nomination form and evaluation criteria separately for each category. We recommend you save your content for each answer as you may want to re-use/re-work it for another Award Category nomination. Additional information may be attached in a word document if more space is required. Nominations are not required for the Earle Duus Award, the recipient of this Award will be decided by the judging panel from the winners of the eight Awards categories listed above.

SECTION 6: EVALUATION CRITERIA (no more than one A4 page per evaluation criteria)

+ All nominations must respond to the evaluation criteria.

+ While there are some common criteria across all categories, each category has its own specific evaluation criteria. Please only fill out the criteria for the award nominated for.

+ Please note: You may choose to respond to the evaluation criteria as a separate document which can be attached in section 7 (Supporting Material).

+ Media nominations: for ease of judging - please provide video content via URL and also on a USB. CD’s cannot be accepted.

Individual Award
  • Demonstrates respect, compassion and caring towards people who live with a mental illness, their families and/or carers.
  • Enhances the quality of life and wellbeing of people who live with mental illness.
  • Promotes social justice and raises mental health awareness in the community.
  • Demonstrates qualities of leadership, vision and innovation.
Media Award
  • Evidence that the story is well researched, balanced and is factually accurate.
  • Promotes a positive attitude towards people with mental illness, their family and/or carers.
  • Raises awareness of mental health issues in the community.
  • Evidence that supports the effectiveness of the publication or broadcast (letters to the editor, talkback radio, emails from the community,etc.).
  • Please provide video content via URL and also on a USB.CD’s can not be accepted.
Youth Award (25 Years And Under)
  • Demonstrates respect, compassion and care towards people who live with a mental illness, their families and / or carers.
  • Enhances the quality of life and wellbeing of people who live with mental illness.
  • Promotes social justice and raises mental health awareness in the community.
  • Demonstrates qualities of leadership, vision and innovation.
Jude Bugeja Peer Experience Award
  • Enhances the quality of life, supports and/or services for people with a mental illness in Queensland.
  • Promotes social justice and raises mental health awareness in the community.
  • Promotes and facilitates consumer and/or carer participation in Queensland.
  • Demonstrates qualities of leadership, vision and innovation.
Not-for-profit Organisation Award Sme (1-100 Staff)
  • Enhances the quality of life and wellbeing of people who live with mental illness.
  • Demonstrates effective collaboration and consultation with consumers, their family and/or carers and other relevant agencies.
  • Delivers best practice services for people who live with mental illness.
  • Demonstrates qualities of leadership, vision and innovation.
Not-for-profit Organisation Award Large (100+ Staff)
  • Enhances the quality of life and wellbeing of people who live with mental illness.
  • Demonstrates effective collaboration and consultation with consumers, their family and/or carers and other relevant agencies.
  • Delivers best practice services for people who live with mental illness.
  • Demonstrates qualities of leadership, vision and innovation.
Workplace Award Sme Businesses (1-200 Staff)
  • Enhances the quality of life, wellbeing and inclusion of people who live with mental illness.
  • Promotes social justice and raises mental health awareness in the workplace and wider community.
  • Delivers best practice programs to promote positive mental health.
  • Demonstrates qualities of leadership vision, and innovation.
Workplace Award Lge Businesses (201+ Staff)
  • Enhances the quality of life, wellbeing and inclusion of people who live with mental illness.
  • Promotes social justice and raises mental health awareness in the workplace and wider community.
  • Delivers best practice programs to promote positive mental health.
  • Demonstrates qualities of leadership, vision and innovation.
EARLE DUUS AWARD

This overarching award is given in recognition of the achievements and success of individuals or organisations which have made significant contributions to people living with mental illness.
The award is given to one of the finalists of the above eight categories in the 2017 Queensland Mental Health Week Achievement Awards.


SECTION 7: SUPPORTING MATERIAL

+ Attach any material that supports the nomination (no more than five A4 pages of attached material).

+ Please note: Supporting material cannot be returned. Do not submit any materials you need returned.

+ If you have more than one file to upload for your application can you please save the files in a zip. folder before uploading.
