Open Minds work together with people to achieve their goals and enhance their mental health and wellbeing.
We offer a range of tailored support, focused on the individual needs of each person. Our supports and services are flexible and everyone is able to decide which supports they require and when they need them. Open Minds is committed to helping every client on their National Disability Insurance Scheme (NDIS) journey and will ensure every individual and their family, carers or guardians are supported through the process. Living, housing, community access, employment and health supports and services can be combined and coordinated to develop individualised and unique service packages.
People already receiving a funded package from the government will continue to receive their current supports until they transition to the NDIS. We will walk the NDIS transition journey with these people who currently have Open Minds services. We are also committed to supporting new clients to explore and determine what the NDIS means for them. We are dedicated to ensuring that each person's journey through NDIS is focused on your life goals, is seamless and collaborative. We will support you throughout the NDIS Planning process and then put your plan into action in a way that suits you.
Our person-centred approach supports people living with an acquired brain injury, disability or mental health issues to find their own strengths and create their own goals. We do this by working alongside individuals in our five support streams: My Home, My Job, My Life, My Health and My Community. At the heart of how we work is our commitment to focusing on individual strengths, enabling recovery and sending ripples of positive change throughout the community. Being part of a community often provides us with a sense of support and understanding, especially when we are going through a difficult time. There are a number of community groups at Open Minds that you or someone that you know can get involved in.

In preparation for the roll out of the National Disability Insurance Scheme (NDIS) in Queensland on the 1st July 2016, Open Minds has been busy implementing our 4 stage readiness project.
The NDIS ensures that people living with a disability are given every opportunity to make their own informed decisions to exercise choice and control, and connect with the community. The NDIS will allow eligible people to receive supports that are reasonable and necessary to reach their goals and we can support you to achieve your NDIS goals and aspirations.

The NDIS will provide a range of funded supports along with information, referrals and linkages to ensure individuals with disability, families and carers are able to access the right supports at home and in the community If you are not quite sure what exactly it is that you are looking for, Open Minds have introduced skilled Service Planners to assist you in choosing the right services and supports for you.
Various Participant Readiness activities have been set up for the NDIS. These include workshops, meetings, forums, online resources, home visits, coaching and mentoring, to ensure that people living with a disability, their families and carers can learn more about the NDIS and be ready for changes as they occur.
