If you are the family or carer of a person who lives with a mental illness, disability or acquired brain injury, we have a number of services centred around: My Home; My Job; My Life; My Health; and My Community to support you and your family member to achieve goals and aspirational wishes. As a family member or carer of a person who lives with a mental illness, disability or acquired brain injury, we will support you along your loved one’s National Disability Insurance Scheme (NDIS) journey. We can support you to ensure that the transition from the current system to the new NDIS system is seamless, or the transition to the new world with supports is as easy as possible and well planned to ensure that goals and needs drive supports.

We offer a range of tailored support, focused on the individual needs of each person. Our supports and services are flexible and everyone is able to decide which supports they require and when they need them. Open Minds is committed to helping every client on their National Disability Insurance Scheme (NDIS) journey and will ensure every individual and their family, carers or guardians are supported through the process. Living, housing, community access, employment and health supports and services can be combined and coordinated to develop individualised and unique supports.
People already receiving a funded package from the government will continue to receive their current supports until they transition to NDIS. We will walk the NDIS transition journey with individuals currently supported by Open Minds services. We are also committed to supporting our prospective and new customers to explore and determine what the NDIS means for them. We are dedicated to ensuring that your journey through NDIS is focused on your life goals and is seamless and collaborative. We will support you throughout the NDIS Planning process onwards to putting your plan into action in a way that suits each customer.
Our people-centred approach supports clients living with an acquired brain injury, disability or mental health issues to find their own strengths and create their own goals. We do this by working alongside individuals in our five support streams: My Home, My Job, My Life, My Health and My Community. At the heart of how we work is our commitment to focusing on individual strengths, enabling recovery and sending ripples of positive change throughout the community. Everyday we work to enable individuals to live a better day, a better week, a better life.
Being part of a community often provides us with a sense of support and understanding, especially when we are going through a difficult time. There are a number of community groups at Open Minds that you or someone that you know can get involved in.
